As a Member of Beech Mountain Club, you have the right to "opt out" of inclusion in photographs and videos which may be used for promotional purposes in publications in any format, including but not limited to print media, the Club’s website, Facebook page and other social media. 
 
If you do not elect to opt out, you are aware that Beech Mountain Club reserves the right to photograph or video members, their families and guests while engaged in activities at Club facilities or while participating in any of our off campus sponsored events.
 
If you wish to opt out, you must check the box below, fill in the following information and click submit. This election shall remain in effect for as long as you maintain your membership in Beech Mountain Club, or unless otherwise stated in conjunction with policy change(s).

By checking the box below, you are saying "PLEASE DO NOT PUBLISH OR IN ANY WAY USE MY IMAGE/PHOTO AND/OR VIDEO FOR PROMOTIONAL PURPOSES."
 

Pursuant to the membership referenced above, list the names of all members to whom this Photo/Video Opt-Out Release Form applies. This Form is not required for minor children under the age of 18 as the Club will not use or make public an image of a child under the age of 18 without the prior, written consent of the child’s parent or legal guardian. To obtain such consent, a Minor Child Image Release and Consent Form must be completed.
