By clicking "Submit" on this Release and Consent Form, you are giving permission to the staff and/or representatives of Beech Mountain Club to take and use photographs, video or sound recordings of your child/children under the age of 18. This is completely voluntary and your consent to the use of the photographs, video and sound recordings and your child’s image, likeness, appearance, and voice may be cancelled at any time with proper written notice. 
 
The Club may use photographs, video and sound recordings containing your child’s image, likeness, appearance and voice in any manner or media, including use on web pages, Facebook and/or other social media for the internal or external promotional activities of Beech Mountain Club. The photographs, video and sound recordings may be used in whole or in part, alone or with other recordings. It is understood that compensation will not be provided now nor in the future. The Club has the right to copy, edit, alter, retouch, revise and otherwise change the photographs, video and sound recordings at the Club’s discretion. All rights, title, and interest in the photographs, video and sound recordings belong solely to the Club.  
 
You further give permission to the Club to use your child’s name, biography, and any other personal data, events, or other material in connection with any such uses of the photographs, video and sound recordings. 
 
I understand and agree to the conditions outlined in this Release and Consent Form. As the parent or legal guardian of the participant named below and on behalf of my child, I give consent to the Beech Mountain Club and its employees/representatives to make use of my child’s image, likeness, appearance, and voice in photographs, video and sound recordings as described above.
