Family Guest Card Application
Family Guest Cards are required for all qualified family members age 10 and up. Please complete this form to submit your request for Family Guests.
Lost cards must be promptly reported to Member & Guest Services. A $20 reprint fee is charged for lost, damaged or stolen cards.
By clicking the "submit" button below, you are applying to sponsor Family Guests desiring to access Beech Mountain Club facilities and services. You are confirming that you have read the Family Guest policies and that you understand you must submit this application 48 hours in advance in order to reserve Family Guest cards for your guests. Family Guest card fees are not refundable.

Members have two options for the purchase of Family Guest Cards and/or access to the Fitness Center.
1. A $15 Family Guest Card issued November 1 through October 31. Optional Fitness Center access* is available through the payment of an additional $5 per day or $25 per week User Fee.
2. A $125 Family Guest Card which includes the card and access* to the Fitness Center for the fiscal year (November 1- October 31).
*The Fitness Center is reserved for Member access only between the hours of 8am and noon in June, July, and August. No guests may access at this time. The Fitness Center is available to guests ages 19 and up. Juniors 14 through 18 years of age are permitted when supervised by an adult. Children age 13 and under are not permitted in the Fitness Center. All Fitness Center users must complete a medical questionnaire and sign a release form prior to use.

I understand that, if opted above, my member account will be charged $15 OR $125 for year-round access for each card requested. I further understand that, in the event the cards are not picked up from Member & Guest Services, my member account will be charged for the cards, regardless of payment option selected above.
